Healthy Connections Medicaid & Medicare

Starting January 2026, South Carolina Healthy Connections Medicaid made important changes for members who receive both Medicaid and Medicare services.

What’s New?
  • Healthy Connections Medicaid members who are entitled to Medicare may now enroll in Healthy Connections Medicaid Managed Care (some exclusions apply).
  • Healthy Connections Medicaid partners with Medicare to combine coverage through Dual Special Needs Plans (D-SNPs).

What Are Dual Special Needs Plans (D-SNPs)?

South Carolina Healthy Connections Medicaid partners with Medicare health plans to offer two types of D-SNPs:

1. Highly Integrated Dual Eligible Special Needs Plan (HIDE D-SNP)
  • A Medicare Advantage plan that provides coverage for long-term services and supports (LTSS) or behavioral health services.
  • Must meet one of the following:
    • Agreement between the Medicare Advantage organization and SCDHHS.
    • Agreement between the parent organization and SCDHHS.
    • CMS designation as a HIDE D-SNP.
2. Coordination Only (CO D-SNP)
  • Designed for individuals eligible for both Medicare and Healthy Connections Medicaid.
  • Focuses on care coordination with the member’s Medicaid Managed Care Organization (MCO).
Highly Integrated Dual Special Needs Plan (HIDE D-SNP)

If you enroll in a HIDE D-SNP, you may have additional enrollment requirements.

In South Carolina:
  • Members who choose a D-SNP for Medicare will also be enrolled in a Medicaid Managed Care Organization (MCO) that aligns with the same parent organization.
  • This is called Exclusively Aligned Enrollment (EAE).

CO D-SNPs do not offer a Medicaid MCO but work with your existing Healthy Connections Medicaid MCO to coordinate care.
